You record areas of agreement, areas of difference and residual uncertainty, citing the guideline or evidence you rely on. Guidance is written for a lay reader and a clinical reader simultaneously.
You digitally sign the report. Your name, specialty and registration jurisdiction appear on the issued document, and the signing event is recorded in the case audit trail.
Reviewers provide independent advisory opinions on documentary evidence. Reviewing a case does not create a treating relationship, and reviewers do not prescribe, order investigations or assume responsibility for the patient's ongoing care.
